How to Make an OMR Sheet for Mock Tests
How to produce practice OMR sheets for your test series, how to check them fast by hand, and the difference between practice sheets and machine-readable ones.

If your students are preparing for JEE, NEET, SSC or any other bubble-sheet exam, the sheet is part of what they are practising.
Filling 180 bubbles under time pressure is a skill. Students lose real marks to it — a row shifted by one, a bubble half filled, four minutes wasted at the end transferring answers from the question paper. None of that has anything to do with whether they know Physics, and all of it is fixable with practice.
Which means running your mock tests on plain ruled paper is training them for an exam that does not exist.
What goes on the sheet
Institute name and test name at the top. You will be handling a stack of these later.
Student details block — name, batch, date, signature, on ruled lines they fill by hand.
Roll number bubble grid. A column of 0–9 bubbles for each digit of the roll number, with a box above to write the digit. This is the part students most often get wrong in the real exam, so it is the part most worth practising.
Answer grid. Numbered questions, each with A B C D (or A to E) as bubbles.
Instructions, one or two lines at the bottom.
Layout decisions that matter
Columns. For a 100-question test, four columns fits comfortably on one A4 page. One long column of 100 needs three pages and is miserable to check. Two columns works for 50 questions.
Bubble size. Big enough to fill with a ballpoint in under a second, small enough that the sheet fits. Around 12–14px on screen prints about right on A4.
Spacing between question rows. This is the one people get wrong. Rows packed too tightly cause students to shade the wrong line, which produces a "wrong answer" that is really a layout failure. Leave a visible gap.
Print at 100%. Turn off "fit to page" and "shrink to fit" in the print dialog. Scaling changes bubble size and spacing, and if you are stacking sheets to check them, they need to be identical.

You have three options.
Draw one and photocopy. Works. Ruling 100 rows of four bubbles by hand once, then photocopying, is a legitimate approach and costs almost nothing. The bubbles will be uneven, which does not matter for practice.
Build one in Excel. Cells as bubbles, borders on, print gridlines. Takes an evening to set up and you can adjust the question count afterwards.

Whichever you pick, print one and fill it in yourself before printing eighty. You will find something — a column too narrow, bubbles too close, roll digits that do not match your numbering.
Practice sheets are not machine-readable sheets
Worth being clear about, because it catches people out.
The sheet you print at home is for manual checking. A real OMR scanner needs specific registration marks in the corners, precise bubble positions, particular paper weight, and often a specific ink colour — all defined by whoever supplies the scanner. A sheet that looks right to a human will not be read by a machine.
For a coaching centre running monthly mocks for forty students, manual checking is genuinely fine and this is a non-issue. If you are running a test series for six hundred students across three branches, that is the point to talk to an OMR vendor about pre-printed sheets and a scanner. Do not expect a home-printed sheet to bridge that gap.
Checking a stack by hand, quickly
The bit nobody explains.
Make an answer key overlay. Take a blank sheet, mark the correct answers, then punch or cut out the correct bubbles. Lay it over a student's sheet and the correct answers show through. Count the marks visible. This turns two minutes per sheet into about twenty seconds.
Check in roll number order. Sort the stack first. It makes entering marks afterwards far faster and stops you losing one.
Do one full manual check as a control. Check one sheet properly by hand and compare against your overlay result. If they disagree, your overlay is misaligned and every sheet you have checked is wrong.
Mark the total on the front. Top-right corner, circled. When you hand sheets back, students want the number and nothing else.
Negative marking
If the exam your students are sitting has negative marking, your mock should too. JEE and NEET both penalise wrong answers, and a student who has only ever practised without it will guess far too freely on the day.
Write the scheme at the top of the sheet — "+4 for correct, −1 for incorrect, 0 for unattempted" — and apply it when you total. Part of what a mock is teaching is when not to answer, and that lesson only exists if the penalty is real.
Afterwards
The sheet gives you a score. The score is worth something only if you keep it.
Three mock scores for a student, in order, tell you whether the preparation is working. One score tells you almost nothing. Most centres lose this because the marks live on the sheets, the sheets go back to the students, and by March nobody remembers what anyone scored in September.
Write the marks down somewhere the same day — a register, a spreadsheet, or Batch Tutor, which keeps test marks per student offline and turns them into a report card when you need one. The method matters less than doing it before the stack goes home.
